- add explicit tooltip placement support to keep chat actions readable
- show project and session row actions on hover for quicker access
- refine sidebar overflow menu sizing and interaction behavior
- avoid showing chat session actions for empty conversations
- remove fallback chat title text so the current session title is used consistently
- bump project version to 1.2.25
