Correct name of the application

In README.adoc I see the following names that relate to this application:

1. Asciidoc Editor
2. Asciidoc Editor and Toolchain
3. Asciidoc FX
4. AsciidocFX

I suggest choosing the current one and correcting the other names to the correct ones.
